At any one time, we help over 750 people living with terminal illnesses to live comfortably in their own homes. When you are referred to us, you will be contacted by a member of the Hospice Community Team who will review your symptoms, pain and medication and work closely with your GP and other healthcare professionals to ensure you and your family are receiving the care and support you need.

We can offer practical assistance in the form of equipment to help you maintain your mobility and independence around the home. We can provide advice on money, benefits and legal issues which may have been worrying you since you became ill. And we can provide emotional and spiritual support to you, your family or close friends, should you or they need it.

Your Hospice Community Team member will encourage you and your loved ones to come to the Hospice to meet the team, take part in our drop-in clinic, meet others in a similar position, or join one of the therapeutic sessions in our Living Well programme
